Here’s what you can expect:

After submitting the form, our office staff will give you a call and email you the New Patient Intake Form. Please complete it as thoroughly and accurately as possible.
Once your intake form is complete, our staff will contact you to schedule your intake appointments. If you don’t hear from us within one business day, feel free to call the office directly to schedule.
Your intake includes two appointments:
First Visit: Consultation and Exam
This one-hour appointment includes a discussion about your concerns, as well as a neurological, orthopedic, and musculoskeletal exam. Dr. Campise will also evaluate your overall body health.Second Visit: Report of Findings
During this appointment, Dr. Campise will review the results of your initial exam and provide treatment recommendations based on his findings.
